Tim O'Neill

Tim graduated with a degree in Medicine from the University of Washington Medical Center, Department of Physical Medicine and Rehabilitation, Division of Prosthetics and Orthotics in 1996. He also holds a degree in Psychology from the University of Washington. O’Neill did his residency in Salem, Oregon at Oregon Orthopedic where he gained dual ABC certification in Prosthetics and Orthotics.
Tim later took a manager’s position with an orthotics and prosthetics provider in the Portland Metropolitan area which included work at OHSU. After five years there, Tim started Evergreen in early 2005. O’Neill has grown Evergreen to five offices since its inception by attracting great practitioners who believe in his vision for Evergreen.
Tim has advanced training in computerized prosthetic technology and specialized socket designs.
In his free time, Tim enjoys spending time with his family which includes coaching his children in football and basketball.
Ron Stout
Ron graduated from University of Washington Medical Center, Department of Physical Medicine and Rehabilitation, Division of Prosthetics and Orthotics in 1997. His first position out of school was as a staff Orthotist with Yakima Orthotics and Prosthetics. After four years he was offered a staff position with Oregon Orthotic Services.
In 2005, Ron became the Manager of Evergreen’s Providence Portland location. He received his certification in prosthetics while with Evergreen in 2007 and has attended many educational courses including Otto Bock C-Leg and AxioBionics Functional Electric Stimulation Wearable Therapy. Stout is known for his high energy and caring approach to providing prosthetic and orthotic solutions. Ron enjoys spending time with his family and going fishing .
Dave Blaisdell
David has been in the prosthetic and orthotic field for over twenty five years. He was educated at Lakewood Community College in prosthetics and orthotics. He started out his career as a technician at Shindlers Inc. Blaisdell then completed his dual ABC certification in Prosthetics and Orthotics at Reisens Evergreen of Spokane Washington and Milo Collier & Associates of Long View Washington.
Shortly after his residency he earned a position with Shiners’ Children’s Hospital as the manager of the prosthetic and orthotic technician staff then as the Manager of the Orthotic Department. Before coming to Evergreen, Blaisdell worked for Oregon Orthotic Services for fifteen years as the Manager of their NW Portland clinic. Blaisdell is currently the Manger of both of Evergreen’s Vancouver clinics. He has presented at several national meetings for the Academy of Orthotists and Prosthetists. David has attended numerous CEU courses including Otto Bock C-Leg and C-Leg Advanced, Otto Bock Myo-Electric Upper Extremity and Durr Fillaure Scandinavian Flexible Socket.
David's hobbies include blacksmithing, wood working, engraving, building flintlock rifles and colonial living history.
Jay Sleeth

Jay completed the Cal. State University Prosthetic Program in 1998 and the Rancho Los Amigos Research & Education Institute Orthotics Program in 2002. Jay’s prosthetics residency was completed at the VA Medical Center in Long Beach, California. There he was involved with treating all levels of amputation as well as prosthetic research on the C-leg which lead to his team winning the prestigious Thranhardt Award that Jay presented at a national meeting. In his free time, Jay enjoys spending time with his family, taekwondo, and golf.
Nate Aleksiewicz
Nate Aleksiewicz (Alec-shev-its) graduated from the Feinberg School of Medicine at Northwestern University Prosthetics-Orthotics Center. Nate completed his prosthetic residency at The Rehabilitation Institute of Chicago and has been involved with high level and multiple limb loss patients. He has special training in upper extremity prosthetics. In his free time, Nate enjoys mountain biking and alpine sports.
Jim Chea
Jim graduated from the University of Washington with a degree in Prosthetics and Orthotics in 2003. He completed his prosthetic residency at the University of Michigan, where he managed every level of amputation working as a key contributor to the multidisciplinary rehabilitation team. Jim also spent five years at Boston Children’s Hospital, where he specialized in treating children with scoliosis, cerebral palsy, plagiocephaly, myelomeningecele and a variety of other pathologies.
Jim enjoys traveling abroad, running and ultimate frisbee. Jim has spent some of his free time in Haiti working with amputees.
Richard Miltenberger
Richard graduated with a degree in Orthotics from Feinberg Medical School at Northwestern University in 2009. He went on to complete his degree in Prosthetics at Northwestern in 2010. In addition, he also holds a degree in Liberal Arts and Culture from New St. Andrews College with an emphasis in classical history and ancient languages. He completed his residency in Orthotics at Thompson’s Custom Orthotics and Prosthetics in Spokane, Washington. He then bought a surfboard and came out West to join Evergreen in 2010 to complete his clinical rotation, and eventually stayed on as a resident Prosthetist. Richard enjoys the problem solving aspect of Prosthetics and Orthotics, and focuses intently on completing the task at hand and creating a quality product for every patient.
Robert Holzman
Rob attended Penn State University and Marywood University in Pennsylvania and went on to become an ABC Certified Pedorthist (C-Ped) through Temple University's school of Podiatric Medicine in Philadelphia. He was a ski coach for 15 years and coached freestyle skiing and inverted aerials at the Olympic Training Center in lake Placid, NY. Working with athletes introduced him to orthotics where he started Pocono Sport Orthotics. In 2003 he went to work as an orthotist for Hanger Orthotics in Central Pennsylvania. He and his wife moved to Spokane, WA in 2008 where he worked for an orthotic and prosthetic facility for 3 years. Rob is certified in Walk-Aid, and has published articles on orthotics and prosthetics as well as a published book on the sport of rock climbing. He and his wife enjoy rock climbing, skiing, wasting time at Powell's Books, and hiking with their dogs Mia and Khloe.
Mark Sauser
Mark is a prosthetist and orthotist certified by The American Board for Certification in Orthotics, Prosthetics and Pedorthics (ABC). Mark obtained his bachelor of Science degree in orthotics and prosthetics from the Washington Medical Center Department of Physical Medicine and Rehabilitation, Division of Prosthetics and Orthotics in 2006. He trained in Spokane, Washington where he had experience providing a full spectrum of prosthetic and orthotic patient care. He now brings a total of 10 years of this specialized professional experience to our office. Mark is married and has three young children. He enjoys family and friends, fishing, woodworking, bicycling and dirt biking.
Cindy Ottley
Cindy Ottley has been with Evergreen since 2007. She received her Associate of Arts degree from Walla Walla Community College. She has vast knowledge and experience in the medical field as she started on this career path at the age of 15. She is responsible for the management of our administrative staff as well as the Billing and Collections. Cindy is most known for her love of anything competitive. She currently participates in Dodgeball, Kickball, Ping-Pong, Bowling, and her newest sport of Kayak Water Polo.
